Volunteer Information & Forms

Anyone interested in coaching or volunteering with PMELL must complete all of the clearances listed below - no exceptions. If we do NOT receive your clearances, you will NOT be allowed to participate in PMELL activities.

1. PA Child Abuse (free) - from Dept Human Services *

2. PA Criminal Record (free) - from State Police *

3. FBI Criminal History Background Check (two options, A or B, only need to do one) *
a. Fingerprint ($22.95)

b. Affidavit Form (free) - only if you've lived in PA for a minimum of 10 consecutive years

* Clearances 1, 2 and 3 (above) are valid for 5 years. Previously obtained clearances, NO OLDER THAN 5 years, are acceptable.

4. Abuse Awareness Training (free) - from Little League **

** #4 (above) must be completed every year.

NOTE: additional training is required for coaches participating in tournament play (i.e. All Stars).
